The Survey was developed (with permission) based on the areas and indicators of organizational effectiveness identified in this report. Is your organization vital and effective? You may find out or affirm what you already know to be true by completing this survey. The survey addresses the indicators of an organization's effectiveness identified within each of the six areas. This is about your organization, so you have the best reason to be honest and objective.

Each area will total your score. The raw data will give you insight into the areas where your organization is most effective and categories where you have opportunities for improvement.

Once you've completed this survey, you may find the Organizational Performance Assessment a useful instrument. This instrument, once completed, helps an organization to examine opportunities that are available to you in order to become more effective.
